The following forms are required annually for all students who participate in TAPPS activities.  All athletes will need a physical and all forms signed BEFORE the first day of practice for their sport.

TAPPS Medical History & Physical Evaluation Forms - TAPPS requires ALL three pages to be completed and signed.

TAPPS Parent and Student Notification Steroid Use Agreement Form

Student Transfer Form (STF) - Parts A and B need to be completed for ALL High School transfer students

Previous Athletic Participation Form (PAPF) - This form must be filled out by parent and signed by athletic director from previous school if athlete is incoming 10th - 12th grade and played sports at a previous school.
